You will be provided with our clinic policy document at your first visit, but the following is information you may need beforehand.
Insurance billing
Dr. Wells is a preferred provider for most major medical insurance companies. It is your responsibility to know the details of your plan, including whether the plan has Naturopathic benefits and covers our providers. You are required to provide proof of insurance coverage (insurance card) at the time of your visit. If your policy has an office visit co-payment, you agree to pay the co-payment at the time of your visit.
Dr Wells participates in the following insurance networks:
- Blue Cross Blue Shield Network
- Bridgespan
- First Choice Health Network
- HMA (Health Management Administrators)
- Kaiser PPO (not Core)
- Lifewise
- Premera Blue Cross
- Regence Blue Shield
- Regence Group Administrators
- Uniform Medical
- Personal Injury and L&I
We believe that everyone should have access to excellent medical care regardless of insurance coverage. We offer a time of service discount for all non-insured patients and a sliding scale for patients who qualify based on income. Please call the office if you have any questions about insurance coverage or sliding scale eligibility.
Financial policies and fee schedule
You can review our Financial Policy Agreement prior to your appointment and please contact us if you have any questions.
Late cancellations and no show fees
To avoid late fees, please cancel your appointment at least ONE business day in advance of your scheduled time.
Patients that “no show” or do not give notice ONE business day in advance of their appointment may be assessed a late cancel/no show appointment charge .
Dr Wells does not double book appointments and therefore, late arrivals of 15 minutes or more may be required to reschedule their appointment and will be assessed a charge for that time.
